Employee Missed Punch Form

You must fill out this form completely for us to fix your missed punch.

If you have missed a punch, please provide the following information and our payroll team will contact you within 48 business hours subject to holidays.

IMPORTANT – Career Start pay period is from Sunday to Saturday. Failure to punch in or out at the designated times during your shift may result in your pay not being guaranteed until approved by payroll.
